Your Privacy is extremely important.  Motherhood Matters is committed to providing you with the best possible customer service experience while protecting your personal and health information.  Motherhood Matters is bound by the Privacy Act 1988, which sets out a number of principles concerning the privacy of individuals as well as the Australian Privacy Principles (APPs).  This Privacy Policy relates to both use of the Motherhood Matters website (www.motherhoodmatters.com.au), the Motherhood Matters Circle Community and the Midwifery and Family Health support services provided by Motherhood Matters.

Collection of your personal information

We collect personal and health information for reasons that include:

  • communicating with you;

  • enabling us to provide services to you;

  • responding to your request for information about our services;

  • sending statements and invoices to you;

  • collecting payments from you;

  • marketing our services to you;

  • dealing with complaints and consumer guarantees;

  • educational purposes;

  • developing, managing, and administering our services; and

  • complying with our legal obligations.

This personal and health information may be obtained from you directly, for example, in person during a clinical consultation, over the phone, by email, or through our website.  We may also collect information from third parties and public sources. 

The type of personal and health information we collect includes your name, address, telephone numbers, e-mail address, date of birth, payment information, transaction information, bank account details, credit card details, Internet Protocol (“IP”) address, server address, domain name, information about your browsing activity, information contained on identification documents and other details that may be required to conduct our services including your health and medical history. 

Use of personal information

We use personal information and health information you provide us to make clinical assessments and develop a management and breastfeeding plan for you and your child/ren. Where necessary, such as in correspondence with your GP or other health professional, your health information may also be used for ongoing management and care.

Other than for the purpose for which you have provided us with your personal information and health information, we may also use it for other related purposes or as permitted or required by law. Such relating purposes include:

  • issuing receipts

  • responding to your comments or questions

  • providing you with marketing material

We may disclose your personal information to the extent that we are required to do so by law or connection with any legal proceedings or prospective legal proceedings involving us.

Disclosure of personal information

We may share any personal information you have provided us during our evaluation and/or management of you and/or your child/children to your other health care providers, as deemed reasonably necessary for your ongoing care and as outlined in your signed consent form.

Any personal information you provide us may also be disclosed, if appropriate, to third parties who provide services on our behalf, such as but not limited to handling customer support enquiries, outsourcing administrative tasks and providing booking and client management platforms.  Those companies will be permitted to obtain only the personal information they need to deliver the service they have been hired to perform on our behalf.  We may disclose your personal information if required by law.

We will only disclose your personal information for the purposes for which it was initially collected, other than directly-related purposes or purposes to which you have otherwise consented.

Data Security 

We are committed to protecting your data.  We take commercially reasonable steps to ensure your personal information is protected from misuse, loss, interference and unauthorised access, modification or disclosure.  

However, data protection and security measures are never completely secure and, despite the measures we have put in place, we cannot guarantee the security of your information.  You must take care to protect your personal information (for example, by protecting any usernames and passwords) and you should notify us as soon as possible of any security breaches.  

Accessing the website

There are many aspects of the website which can be viewed without providing personal information, however, for access to certain features you are required to submit personally identifiable information. This may include but not limited to a unique username and password, or provide sensitive information in the recovery of your lost password.

Cookies

When you visit our website, we will collect and store your computer’s assigned IP address, the date and time of your visit, the information accessed and the referring page. We will also collect other non-identifying data for statistical purposes. This information is not linked to your personal information unless required by law.

Like many websites, our website may use ‘cookies’ from time to time. Cookies are small text files that the website transfers to your computer through your web browser to enable the website’s systems to recognise your computer. Cookies may also be used to record non-personal information such as the date, time or duration of your visit, or the pages accessed, for website administration, statistical and maintenance purposes. Motherhood Matters is not responsible for the privacy practices or the content of such websites, which are governed by the privacy policy of third parties. Motherhood Matters is not in a position to verify the accuracy of the information or any claims made by third parties.

You may seek access to the personal information we hold about you.  We may restrict access where we are required or permitted to do so by law. 

Accuracy, completeness and up-to-date information:

We will take reasonable steps to ensure that the personal information we hold is accurate, complete and up-to-date.  You may contact us to update your personal information or tell us that the personal information we hold about you is inaccurate, incomplete or out-of-date.

We reserve the right to make changes to our Privacy Policy at any time.  We may update this Privacy Policy by posting an updated version on our website (www.motherhoodmatters.com.au).

Terms of Service

By making this booking you consent to working with me, Felicity Hughes, an International Board Certified Lactation Consultant (IBCLC).

You give permission to work with me and the Motherhood Matters team during this consultation for your breastfeeding, feeding, sleeping problem/concern.

This consent is for ;

- face-to-face consults

- virtual visits

- phone consults

-interaction in the Motherhood Matters Community

- all follow-up including phone conversations, email, text and information sent via Circle Community, the internet or regular mail. You understand that I am an Allied Health Care Provider and not a Medical Practitioner and that if there are any concerns or any reason to believe there is any underlying illness of any kind then support and referral to a medical practitioner should be sought.

Motherhood Matters is committed to the education, training and development of aspiring, future IBCLCs and to the support and development of other health care professionals in the infant feeding and development field. You give consent for the lactation consultant to use clinical information obtained during our sessions for education of other health care providers and students about lactation and infant development. You and your baby will not be identified by name, but aspects of your situation might be described and discussed.

You give permission for photographs and recordings to be made for charting, documentation and education purposes. If the photographs and/or parts of recordings are to be used for education and training, a pseudonym will be used to protect your privacy. All educational recordings will be owned by Motherhood Matters and stored in a secure platform, separately from client files (which are not accessible to students at any time), to ensure your privacy is protected at all times. You understand that these images and recordings will be used for the purpose of education and the promotion of breastfeeding and lactation counselling.

You understand that all educational material will be accessed via a secure, password protected site that is managed solely by Motherhood Matters with no 3rd party involvement. You understand that if you do not wish to be photographed or recorded or if you would like more information about the education program, you can let the Practice Manager know at the time of this booking via the notes section or by emailing info@motherhoodmatters.com.au.

You understand that for lactation consultations and all follow-up, interaction in the Community, the lactation consultant will protect the privacy of your personal health information as required by the Code of Professional Conduct of the International Board of Lactation Consultant Examiners, the IBLCE Scope of Practice for IBCLCs and as per the Australian Privacy Principles.

You take full responsibility for implementing any suggestions that may be made during our work together. You understand that my advice is limited to providing options for your consideration and that you are solely responsible for any actions that you choose to take. Always consult your own values and vision, do your own research and check with other appropriately qualified professionals before making major decisions or making significant changes. You agree to indemnify me and the Motherhood Matters team against all consequences arising directly or indirectly from your choices.
